Contrast-Enhanced Mammography (CEM) is a special type of mammogram that uses an iodine-based contrast dye and X-rays to provide more detailed images of your breast tissue. Why is Contrast-Enhanced Mammography (CEM) Performed?

Your doctor may recommend Contrast-Enhanced Mammography (CEM) if:

  • You have a newly diagnosed breast cancer and additional imaging is needed.
  • A suspicious area was found on your screening mammogram or ultrasound.
  • You cannot undergo a breast MRI due to claustrophobia, a pacemaker, or other reasons.
  • More detail is needed after surgery or radiation treatment.

How to Prepare for CEM Exam

  • Contrast Dye: You will receive an iodine-based contrast injection in your arm before the mammogram images are taken. Tell your doctor if you have ever had an allergic reaction to contrast dye.
  • Food & Drink: You may eat and drink normally before your appointment, unless instructed otherwise.
  • Medications: Continue taking your usual medicines.
  • Kidney Function: Some patients may need a simple blood test before the exam to make sure their kidneys can safely process the contrast.
  • Clothing & Jewelry: Wear a two-piece outfit so you can undress from the waist up. Avoid powders, deodorants, or lotions on your chest or underarms the day of the exam, as these can affect image quality.
  • Pregnancy: Let your doctor know if you are pregnant or think you may be pregnant.

What to Expect During the Exam

  • A technologist will place an IV in your arm for the contrast injection.
  • After the injection, you will have a standard mammogram with breast compression, just like a routine exam.
  • Both low- and high-energy X-ray images are taken, and then combined by a computer to highlight areas of concern.
  • The entire process usually takes less than 15 minutes.

After the CEM Exam

  • You can return to normal activities right away.
  • Drink extra fluids to help flush the contrast from your body.
  • Your doctor will receive the results and review them with you.
